Hashimoto’s Thyroiditis

Hashimoto’s thyroiditis — the autoimmune cause of most hypothyroidism — adds an additional consideration:

Inflammation connection:

Hashimoto’s is an inflammatory autoimmune condition. GLP-1 medications have anti-inflammatory properties — and some evidence suggests they may reduce inflammatory markers in autoimmune conditions. Whether this translates to meaningful benefit in Hashimoto’s specifically is not established but is biologically plausible.

The practical situation:

People with Hashimoto’s on adequate levothyroxine replacement are in essentially the same position as people with non-autoimmune hypothyroidism — the thyroid condition managed, the GLP-1 treatment proceeding as for any patient with controlled hypothyroidism.


Hyperthyroidism and Weight Loss Injections

Hyperthyroidism — excess thyroid hormone — creates a different set of considerations.

Active Untreated Hyperthyroidism

Active hyperthyroidism significantly complicates weight management:

  • Elevated metabolic rate initially causes weight loss (not gain)
  • But muscle wasting alongside weight loss is significant
  • Cardiac effects (tachycardia, arrhythmia) are potential concerns with any medication affecting metabolism

For people with thyroid conditions — monitoring during GLP-1 treatment:

For hypothyroidism on levothyroxine:

  • TSH check 6-8 weeks after starting GLP-1 medication
  • Thereafter, follow your usual monitoring schedule (typically every 6-12 months when stable)
  • Dose adjustment of levothyroxine may be needed if TSH shifts

For all GLP-1 users:

  • Annual TSH as part of general health monitoring
  • Prompt evaluation of any new neck symptoms (lump, hoarseness, pain)
  • Thyroid ultrasound only if clinical indication — not routine screening

The Thyroid Cancer Risk in Real Numbers
